She specializes in treating patients with psychotic disorders, bipolar disorders, depressive disorders, and anxiety disorders. She was previously a faculty member at the University of Maryland Medical Center, and continues to teach residents and medical students as well as can be heard on a variety of podcasts providing educational topics of the impact of lifestyle on cognitive health. In addition to his role at Greenbrook Mental Wellness Centers, Dr. Sargent is the CEO and Medical Director at Peak Health Services in Oceanside, CA. Here, he leads a dynamic team focused on improving access to high-quality mental health care. He also provides psychiatric coverage for inpatient hospitals and substance programs while consulting with federal and aviation agencies on mental health evaluations, fitness for duty and development of specialty programs. His expertise extends to human performance training for various professionals, including athletes and first responders and has also been instrumental in developing the U.S. Navy's first residential PTSD treatment program and has served as a Group Psychiatrist for Naval Special Warfare.

With board certifications in Psychiatry and Neurology, Brain Injury Medicine, and specialized training in modalities such as Transcranial Magnetic Stimulation and Medical Acupuncture, Dr. Sargent brings a wealth of knowledge and experience to his practice. Growing up in Louisiana, attending college and medical school in Tennessee, completing his medical residency in South Carolina, and working in Florida, Dr. Poole is known for his unique combination of medical know-how and southern hospitality. His areas of interest include depression, anxiety, smoking cessation, and neuromagnetic medicine. He also has a passion for research. His work includes creating an automated system to help reduce tobacco deaths, improving the informed consent process for controlled medications, and helping to design a brain-computer interface device which allowed patients with Lou Gehrig’s disease and other neurodegenerative diseases to communicate. Q. What is SPRAVATO®?

A. SPRAVATO® is an FDA-approved nasal spray. It is a prescription medicine used:

with or without an antidepressant taken by mouth, to treat adults with treatment-resistant depression (TRD)
with an antidepressant taken by mouth, to treat depressive symptoms in adults with major depressive disorder (MDD) with suicidal thoughts or actions.

Q. Do I need to go to a hospital?

A. SPRAVATO® is available through the SPRAVATO® Risk Evaluation and Mitigation Strategy (REMS) Program at convenient outpatient Greenbrook Mental Wellness Centers. The medical staff at Greenbrook’s REMS-certified outpatient treatment centers are trained to prescribe and observe the patient's self-administration of SPRAVATO®.

Q. What happens after my consultation?

A. Your Care Team will explain payment options, review standard insurance plan criteria, and coordinate with your health insurance provider to determine benefits. Next, you will meet with a Greenbrook-affiliated licensed mental health provider for an evaluation, which includes detailed questions about your history and symptoms. If the provider determines that you are a candidate for SPRAVATO® and you are ready to move forward, they will create your individualized treatment plan.

Q. What is a typical treatment plan?

A. A typical treatment plan consists of:

Induction, Week 1-4: 2 treatments a week
Maintenance, Week 5-8: 1 treatment a week
Maintenance, Week 9+: 1 treatment every 1-2 weeks

Treatment plans may vary depending on clinical conditions.

Q. Do I need to be on medication?

A. SPRAVATO® can be administered as a standalone therapy or combined with other oral antidepressant medications. Our psychiatric professionals will work with you to determine the best approach for your needs.

Q. Does my insurance cover SPRAVATO®?

A. Many major insurance companies cover SPRAVATO® if you haven’t found adequate relief after trying two oral antidepressants. A benefit of having treatment at a Greenbrook center is that our Care Team does nearly all the work for you regarding insurance. We assist all patients with the insurance coverage and the insurance reimbursement process.

Mechanism of Action

SPRAVATO® (esketamine) nasal spray is the S-enantiomer of racemic ketamine. Esketamine, the S-enantiomer of racemic ketamine, is a non-selective, noncompetitive antagonist of the N-methyl-D-aspartate (NMDA) receptor, an ionotropic glutamate receptor. The mechanism by which esketamine exerts its antidepressant effect is unknown. The major circulating metabolite of esketamine (noresketamine) demonstrates activity at the same receptor with less affinity.

It is important to note that the relationship between this pathway and the clinical effects of depression have not been established.

The precise mechanism of action is unknown. ^7-11

The Typical Treatment Experience

No-Cost Consultation

You will meet with a Consult Coordinator to learn about the NeuroStar TMS treatment process, and our Care Team will start the insurance paperwork if needed.

Pre-Assessment

You will have an office visit with a Greenbrook-affiliated provider to evaluate whether NeuroStar TMS is right for you and to answer any additional questions you may have about the treatment.

Motor Threshold

Your Greenbrook-affiliated provider will determine your prescribed dose and treatment location for NeuroStar to deliver the best possible outcomes for you.

Obsessive Compulsive Disorder Can Severely Affect Daily Life

Obsessive Compulsive Disorder (OCD) affects an estimated 2.5 million Americans each year. ¹ OCD can severely affect daily life, routines, and cause distress or even functional impairment.
